15-Minute Initial Consultation: Free
Initial Diagnostic Intake Evaluation (60 Minutes): $275
Ongoing Therapy Sessions (50 Minutes): $225
Private-Pay Therapy & Out-of-Network Benefits
Cardinal Hope and Healing LLC is a private-pay, out-of-network practice. Choosing to pay out of pocket can offer greater flexibility and privacy in how we approach your care.
Greater Privacy: Using insurance typically requires a mental health diagnosis and sharing certain clinical information with your insurance company. With private pay, you can avoid submitting your therapy claims to insurance and have greater control over what information is shared for reimbursement purposes.
You Control Your Care: Without insurance requirements determining what services are covered, we can collaborate on the pace, frequency, and duration of therapy based on your needs, goals, and progress.
A More Personalized Approach: Private-pay therapy allows us to focus on your individual goals and therapeutic needs rather than tailoring treatment to insurance coverage requirements.
If you have out-of-network benefits, you may be eligible for reimbursement from your insurance company. Cardinal Hope and Healing LLC can provide superbills to help you submit claims. Reimbursement varies by plan, so I recommend contacting your insurance provider to confirm your out-of-network benefits.
Using Out-of-Network Benefits
I do not bill insurance providers directly. However, if your plan includes out-of-network mental health benefits, you may be eligible for partial reimbursement for therapy services.
Pay at the Time of Your Session: You pay the full session fee at the time of your appointment. Major credit cards, HSA, and FSA cards are accepted.
Receive a Superbill: I provide a monthly superbill—a detailed receipt that includes the information and billing codes needed to submit an out-of-network claim.
Submit for Reimbursement: You submit the superbill directly to your insurance company according to their claims process. Your reimbursement will depend on your specific plan, deductible, and out-of-network benefits. Reimbursement is not guaranteed.
Tip: Before beginning therapy, contact your insurance provider to ask about your out-of-network mental health benefits, deductible, reimbursement rate, and any claim submission requirements.

To find out exactly what your insurance plan will cover, call the Member Services number on the back of your insurance card and ask these specific questions:
"Do I have out-of-network mental health benefits for outpatient office visits?"
Why it matters: Confirms if your plan allows out-of-network reimbursement at all.
"Are telehealth sessions covered under my out-of-network benefits?"
Why it matters: Since this practice is 100% virtual, your plan must explicitly cover online therapy.
"What is my out-of-network deductible, and has any of it been met yet?"
Why it matters: You must hit this dollar amount out-of-pocket before insurance starts paying you back.
"What is my reimbursement rate or co-insurance percentage for CPT codes 90834 and 90837?"
Why it matters: These are the standard codes for individual therapy. This tells you the exact percentage (like 60% or 80%) you will get back.
"What is the 'Allowed Amount' or maximum cap for CPT code 90837?"
Why it matters: Some plans limit their payback to their own internal fee cap rather than your actual session cost.
"How do I submit my monthly Superbills to request reimbursement?"
Why it matters: Tells you if you can quickly upload your document via an online portal/app, or if you need to mail a physical form.

Please cancel or reschedule at least 24 hours before your appointment time.
Late Cancellation & No-Show Fees
Cancellations under 24 hours—and missed sessions—incur the full standard session fee ($160, $180, or $225).
Please note: Insurance plans do not reimburse or cover fees for missed appointments.
Late Arrivals
If you are running late, please send a quick message to let me know.
We will meet for the remainder of your time, but the full session fee still applies.

Under the No Surprises Act, clients who do not use insurance or who choose to pay privately have the right to receive an estimate outlining the total expected cost of their non-emergency mental health care before services begin.

Calculated based on your unique clinical plan and expected session frequency.
Flexibility & Changes
Actual long-term costs may vary depending on your individual therapeutic goals and progress.

The Right to Dispute
If you receive a bill that is at least $400 more than your provided estimate, federal law gives you the right to formally dispute the charges.
